Important updates to supervisor nomination procedures. Learn about changes to the nomination form and new requirements for dental supervision arrangements effective May 16, 2019.
ATTENTION TO: ALL CONDITIONALLY-REGISTERED DENTISTS AND PART II ORAL HEALTH THERAPISTS AND SUPERVISORS
A revised Supervisor Nomination Form for Conditionally-registered Dentists and Part II Oral Health Therapists has been uploaded, and can be found at here.
With effect from 15 February 2019, only the revised Supervisor Nomination Form will be accepted and processed by SDC. Should you have any queries, please contact SDC at SDC@spb.gov.sg. Thank you.
