Renaming of Supervisory Nomination Form to Letter of Undertaking wef 7 Oct 2020

Important update on supervisory documentation changes. Learn about the renaming of Supervisory Nomination Form to Letter of Undertaking and how this affects your submissions.

ATTENTION TO: ALL CONDITIONALLY-REGISTERED DENTISTS AND PART II ORAL HEALTH THERAPISTS AND SUPERVISORS

The Supervisor Nomination Form has been renamed to the Letter of Undertaking with effect from 7 October 2020.

The Letter of Undertaking for Conditionally-registered Dentists, Part II Oral Health Therapists and supervisors have been uploaded, and can be found here.

With effect from 7 October 2020, only the Letter of Undertaking will be accepted and processed by SDC. Should you have any queries, please contact SDC at SDC@spb.gov.sg. Thank you.
