| When the application is approved, applicant will be requested to call at the SDC's office personally, bringing the following for formal restoration: |

| (a) | original copies of the documents submitted earlier; |
| (b) | identity card/passport; and |
| (c) | prescribed restoration and practising certificate fees. |
